Emergency Relief Assistance

  • DD 214 showing honorable service

  • Proof of household income from the last 30 days

  • Bank transactions from the last 30 days

  • Current utility bills

  • Lease agreement

  • Landlord name and address

  • Other monthly bills

  • Marriage certificate, when applicable

  • Birth certificates or adoption papers for dependent children, when applicable​

Widow’s / Surviving Spouse Pension

  • DD 214 or military separation papers

  • Marriage certificate and divorce decrees, when applicable

  • Direct deposit information

  • Veteran’s death certificate

Service-Connected Compensation

  • DD 214 or military separation papers

  • Marriage certificate, divorce decrees, birth certificates and Social Security numbers for dependents, when applicable

  • Diagnosis and Medical Records

  • Direct deposit information

Non-Service-Connected Disability Pension

  • DD 214 or military separation papers

  • Medical provider information
