  • The Allen County Veterans Service Commission transportation program exists to help eligible Allen County veterans access VA-directed services and non-VA medical services within Allen County.

  • Transportation is provided based on VA-directed appointments receiving scheduling priority. Non VA-directed medical transportation will be limited to within Allen County.

Eligibility Requirements

To utilize transportation services, veterans must:

  • Be an eligible veteran residing in Allen County, Ohio

  • Provide proof of veteran status

  • Provide proof of Allen County residency

  • Complete and sign the required registration and liability waiver forms

​

The Transportation building is located at 201 E North St. Lima, Ohio 45801. The building is specifically for transportation services only. All other veteran related services are still conducted in our administrative office located at 301 N. Main St. Lima, Ohio 45801.

VA-Directed Appointments

Transportation may be available for:

  • Dayton VA Medical Center

  • VA clinics and supporting medical centers

  • VA-authorized Community Care appointments

  • Other VA-directed locations on a case-by-case basis depending on availability

Non-VA Medical Appointments

Transportation for non-VA medical appointments is limited to appointments within Allen County, Ohio only.

Transportation is NOT provided for:​

  • Personal errands (grocery , retail, etc)

  • Non-medical transportation needs

Scheduling Transportation

Veterans are encouraged to leave transportation requests on the transportation voicemail system.

When leaving a message, please speak clearly and provide:

  • Full name

  • Last four digits of SSN

  • Appointment date and time

  • Appointment location

  • Phone number

  • Pickup address if transportation is needed from home

Scheduling Notice Requirements

  • Dayton transportation requires at least 24-hour notice

  • Local transportation requires at least 48-hour notice

Transportation is scheduled based upon availability.

Transportation Guidelines

  • Transportation services are curb-to-curb only

  • Drivers cannot provide medical assistance or physical lifting assistance

  • No food, drinks, or tobacco products are permitted in transportation vehicles

  • One caretaker or caregiver may accompany the veteran if seating capacity allows

ADA Transportation

If ADA-compliant transportation accommodations are required, the Allen County Veterans Service Commission will coordinate transportation services through Regional Transit Authority (RTA) or other partnering agencies based upon availability.

Additional Information

Transportation services are not emergency medical transportation.
